NOTE: Submit the required Quest Informed Consent for Genetic Testing with the specimen if the ordering medical facility is located in the following states:
AK, DE, FL, GA, IA, MA, MN, NV, NJ, NY, OR, SD or VT.
Test is affected by gross hemolysis but is not affected by lipemia.
